What should you know about full term premature rupture of membranes?

The membranes are the fluid-filled sac around the baby. At full term, they can rupture before contractions begin, causing a gush or a continuing trickle of fluid. Urine and vaginal discharge can look similar, so the maternity team may check the fluid, your temperature, the baby’s heart rate and your stage of pregnancy. The fluid may be clear, pinkish or lightly blood-stained; green or brown fluid needs immediate attention because it may contain meconium.

Call your maternity unit or obstetric team as soon as you notice a gush or trickle. Note the time, colour and smell, and use a maternity pad rather than a tampon. Avoid sex, swimming and putting anything in the vagina until you have been assessed. Follow instructions about monitoring and labour. Depending on your examination and clinical circumstances, the team may discuss waiting for labour or induction of labour.

Go to maternity assessment urgently for heavy bleeding, green or brown fluid, fever, feeling unwell, severe abdominal pain, reduced baby movements, or regular painful contractions. Call emergency services if you feel faint, have severe bleeding, or something cord-like is coming through the vagina. Do not drive yourself if you feel unwell.

Ask how the fluid was confirmed, how you and the baby will be monitored, what would change the birth plan, and how long it is reasonable to wait for labour. Ask whether induction of labour or any other treatment is recommended for your situation.
