The short answer
A bacterium called Bacillus cereus is associated with rice that has been cooked and then cooled or stored poorly. Some forms cause nausea and vomiting relatively soon after eating; others cause watery diarrhoea and cramping later. Meat, eggs, vegetables or sauces added to fried rice can also carry other food-poisoning germs if they are undercooked or handled unsafely.
What it means
Typical symptoms include nausea, vomiting, abdominal pain, loose stools and sometimes fever. Symptoms may begin within hours or take longer, depending on the germ or toxin involved. Most uncomplicated illness improves as the body clears the cause, but repeated vomiting or diarrhoea can remove water and salts faster than you can replace them.
The smell or taste of rice is not a reliable safety test. Cooking may kill some bacteria, but toxins already formed in food may remain. Children, older adults, pregnant people and anyone with a weakened immune system can become unwell more quickly from fluid loss and should seek advice sooner.
What to do at home
Take frequent small sips of water or an oral rehydration solution, especially after vomiting or a loose stool. Oral rehydration therapy replaces both fluid and salts. Rest, and eat small amounts of plain food when you feel able; avoid alcohol and foods that clearly worsen nausea. Do not prepare food for other people while you have vomiting or diarrhoea, and wash your hands carefully with soap and water.
For prevention, refrigerate leftover rice promptly in a shallow container, keep it cold, and reheat it thoroughly only when you plan to eat it. Discard rice that has been left out for a prolonged period rather than relying on reheating to make it safe.
When to seek medical care
Arrange medical advice if you cannot keep fluids down, have signs of dehydration such as very little urine, marked dizziness or unusual drowsiness, or have diarrhoea that is severe, persistent or bloody. Seek urgent care for severe or worsening abdominal pain, confusion, fainting, breathing difficulty or a high fever with a rapidly worsening illness.
Get advice early if the person affected is a young child, an older adult, pregnant, or living with a condition or treatment that weakens immunity. A clinician may assess hydration and decide whether you need supervised oral fluids or IV fluid therapy.
