First-Trimester Bleeding After Sex

Pregnancy increases blood flow to the cervix, which can make light spotting after penetration more likely. Irritation, infection, or changes unrelated to sex can also cause bleeding. Bleeding in early pregnancy may present with no serious complication, but it can also occur with miscarriage or ectopic pregnancy. The timing, amount, colour, pain pattern, and pregnancy history help a clinician decide what checks are needed.

Stop sex and use a sanitary pad so you can observe the amount and colour; do not use a tampon. Contact your maternity or early-pregnancy team for advice, even if the bleeding is light and settles. Note when it began, whether clots appeared, and whether you have cramps, shoulder-tip pain, weakness, or dizziness. Avoid inserting anything into the vagina until a clinician advises you about next steps.

Call emergency services or attend emergency care for severe or one-sided lower abdominal pain, shoulder-tip pain, fainting, marked dizziness, heavy bleeding, or feeling very unwell. These symptoms can occur with ectopic pregnancy, which requires urgent assessment. Do not wait for pain to become severe if you have pregnancy bleeding and feel faint. A clinician may use examination, pregnancy testing, and ultrasound to locate the pregnancy and assess your wellbeing.

Ask whether you need an examination, repeat pregnancy testing, or an ultrasound, and which symptoms should send you straight to emergency care. Tell the team your pregnancy dates, blood group if known, bleeding amount, and pain location.
