Fever with a red rash on the face

Note whether the skin is hot, swollen, itchy, scaly, painful or blistered. Redness around one area that feels warm and tender can occur with cellulitis. Some viral illnesses produce a flushed or blotchy face alongside runny nose, cough or sore throat; chickenpox can begin with spots before blisters appear. A new cream, medicine or food may trigger irritation or an allergic reaction, although fever makes infection worth considering.

The face can swell with allergy or infection. Swelling of the lips, tongue or throat, especially with breathing trouble, is an emergency. Do not rely on colour alone, because darker skin may show redness less clearly; feel for warmth and look for changes in texture and spread.

Keep the skin cool and clean, avoid rubbing it, and stop newly introduced cosmetics until the cause is clearer. Encourage fluids and rest. Record the fever, the first location of the rash, recent medicines, foods, insect bites and contact with anyone unwell. Do not apply steroid, antibiotic or other medicated creams to a child's face unless advised by a clinician.

Call emergency services for breathing difficulty, throat or tongue swelling, collapse, confusion, severe drowsiness, seizure, stiff neck, or a rash that does not fade when pressed. Seek same-day advice if facial swelling is increasing, the skin is very painful or rapidly spreading, the eyes are affected, or drinking is difficult. A fever that persists or a person who is worsening also needs medical review.
