Fever and teary eyes: what to know

Watery eyes may accompany a cold or COVID-19 when the nose and tear ducts become inflamed. Fever is a common presentation of several infections, while teary eyes alone can also follow smoke, dust, allergies, or irritation. Chickenpox may cause fever with an evolving skin rash, and Croup may add a barking cough or noisy breathing in a child. Bronchiolitis is more relevant in babies with cough, wheeze, and breathing effort. The combination does not identify one cause without considering age, duration, exposure, and the full pattern of symptoms.

Rest and drink regularly. Gently clean tears with clean water and a fresh cloth for each eye, and wash hands before and after touching the face. Do not share towels, eye drops, or cosmetics. Avoid rubbing the eyes and move away from smoke or other irritants. Follow label directions for fever relief, and ask a pharmacist before using eye medicine, particularly for a child or contact-lens wearer.

Seek urgent care for severe eye pain, light sensitivity, reduced or blurred vision, an eye injury, swelling around the eye, or difficulty breathing. Prompt review is needed for thick discharge, a swollen eyelid, fever in a very young infant, or a child who is difficult to wake. Do not wear contact lenses while the eye is red or watering, and seek advice if symptoms are worsening rather than settling.

Ask whether the eye symptoms fit a viral illness, allergy, or local eye problem, and whether testing is needed. Tell the clinician about contact-lens use, new products, rash, cough, breathing noise, vision changes, and anyone nearby with similar symptoms.
