Bowen's disease, also called squamous cell carcinoma in situ, is an early form of abnormal change confined to the outer skin layer. Around an eyelid, it may look like a persistent scaly, red, brown, or sharply outlined patch and can resemble cosmetic eyeliner. Similar appearances can come from irritation, pigmentation, sun-damaged skin, or other lesions. A photograph or description alone cannot confirm the diagnosis; examination and sometimes a small biopsy are used when the appearance is uncertain.

Do not cover a persistent mark with makeup or try to remove it at home. Book a dermatology review, especially if the area is scaly, crusted, tender, enlarging, or repeatedly returns. Bring a list of creams and cosmetics used near the eye and mention any previous skin lesions or immune problems. If Bowen's disease is confirmed, treatment may include carefully selected topical medicine, cryotherapy, or photodynamic therapy; the choice depends on location, thickness, and proximity to the eye.

Seek prompt assessment for bleeding, ulceration, rapid change, persistent soreness, loss of eyelashes, or a lesion on the eyelid margin. Urgent eye care is appropriate for vision change, severe eye pain, marked swelling, or difficulty opening the eye. These warning signs do not establish cancer, but they make delay unhelpful.
