What the tiredness may mean

Energy can vary during the menstrual cycle, but exhaustion that interferes with work, movement, concentration, or usual self-care should not be dismissed as a normal cycle change. Sleep disruption, stress, low food or fluid intake, pain, heavy bleeding at another point in the cycle, thyroid problems, infection, anaemia, and medicines can all contribute. Keep a brief diary for at least one cycle: sleep, meals, fluids, activity, bleeding, pain, medicines, and the intensity and timing of fatigue. Aim for regular meals, gentle activity as tolerated, and a consistent sleep routine. Do not start iron or other supplements solely because you feel tired; testing can show whether a deficiency is present. Arrange a primary-care appointment if the pattern is new, worsening, or limiting daily life.

Seek urgent help for severe breathlessness, chest pain, fainting, new confusion, sudden weakness, uncontrolled bleeding, or a high fever with marked illness. Contact a clinician promptly for fatigue with unexplained bruising, repeated infections, swollen glands, drenching night sweats, or ongoing unintentional weight loss.
