Extreme pain in shin: what to know

Short answer
Extreme shin pain should be assessed promptly when it limits walking, follows injury, or persists at rest.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pause
- Running, jumping, and repeated impact
- Track
- Exact tender spot, swelling, and pain at rest
What it may mean
The shin contains the tibia, muscles, tendons, nerves, and blood vessels. Pain can follow a sudden impact, a rapid increase in running, repeated jumping, or footwear and training changes. A stress fracture can produce a focused tender area and pain that becomes more noticeable with loading. Shin pain can also reflect muscle irritation or inflammation around the bone. The location, tenderness, swelling, and whether pain settles with rest are useful clues, but they cannot confirm the cause alone.
Pain at rest or at night, pain in one precise spot, or worsening symptoms deserve more caution than mild soreness after unfamiliar exercise.
What to do today
Stop impact exercise and do not test the leg by repeatedly hopping or running. Keep walking only as much as is comfortable, and use a support if needed. Elevation and a wrapped cold pack may ease swelling after an injury. Arrange prompt examination if pain is severe, localised, or not improving. A clinician may assess your gait and bone tenderness and decide whether imaging is needed.
When to seek urgent care
Go to urgent care after a major blow or twist, if the leg looks deformed, or if you cannot bear weight. Seek emergency help for a cold or pale foot, new numbness or weakness, rapidly increasing swelling, severe pain out of proportion to the injury, or fever with redness and warmth.
Questions for your doctor
Ask whether the pain pattern suggests a stress fracture, whether imaging is needed, what level of walking is safe, and how to return to activity without a sudden load increase.

Questions people ask
Can I keep exercising with extreme shin pain?
Avoid impact exercise until the cause is assessed, particularly when pain is focused, worsening, or present while resting.
Does shin pain always mean a stress fracture?
No. Shin pain has several possible causes; a stress fracture is one possibility that needs clinical assessment rather than self-diagnosis.
