What should you know about extreme groin pain?

Short answer
Extreme groin pain should be assessed promptly when it prevents walking, follows an injury, or comes with swelling, fever, or testicular symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pain triggers
- Running, turning, hip flexion, lifting, or coughing
- Associated clues
- Catching, bulging, bruising, fever, urinary, or testicular symptoms
- Assessment
- Hip, abdomen, pelvis, strength, and movement may all be checked
What it may mean
The groin is where the abdomen, pelvis, hip, and upper thigh meet, so pain there has several possible sources. A muscle or tendon strain may follow sprinting, kicking, a sudden change of direction, or heavy lifting. Hip impingement can present with groin pain during bending or rotation, while a hip labral tear can cause catching, clicking, or deep pain. Hip osteoarthritis may make walking, stairs, or turning difficult. Hernia, urinary, reproductive, or abdominal problems can also feel like groin pain and need a suitable medical examination.
What to do now
Stop the activity that provokes the pain and use comfortable, short steps rather than pushing through a limp. Rest from sprinting, kicking, deep squats, and heavy lifting until the cause is clearer. A wrapped cold pack may help after a recent strain. Do not begin aggressive stretching when pain is sudden or severe. Assessment may include hip and abdominal examination, strength and movement checks, and imaging or referral when indicated. Sports physiotherapy can help when a movement-related muscle or hip problem is confirmed.
When to seek care
Seek emergency help for severe pain with fainting, repeated vomiting, a rigid or very tender abdomen, a painful groin bulge that will not go back in, or sudden severe testicular pain. Arrange urgent assessment for fever, inability to bear weight, major bruising or swelling, blood in urine, or pain after a significant injury. New pain during pregnancy should be discussed with the maternity team.
Questions for your doctor
Ask whether the hip, abdominal wall, adductor muscles, or another system seems responsible. Ask which movements to avoid, whether imaging is needed, how to manage a limp, and when a gradual return to exercise is reasonable.
