Extreme diaper rash in a newborn needs gentle skin care and prompt medical advice when it is severe, spreading, or not improving. A bright-red, raw, swollen, or oozing area can result from prolonged contact with urine or stool, frequent loose stools, friction, or irritation from wipes and products. Yeast or another skin problem can also be involved, so a clinician should assess a rash that looks unusually extensive or painful.

Irritant nappy dermatitis often affects the skin covered by the nappy and may spare the creases. A yeast rash may involve the folds and show small spots around the main red area. Broken skin increases the chance of infection and makes wiping more uncomfortable. Newborn skin can change quickly, and a rash can be harder to judge when there are blisters, pus, bleeding, crusting, or marked swelling. Do not assume the appearance identifies the cause; treatment depends on the examination.

Change the nappy promptly after wetting or soiling. Rinse with lukewarm water or use a fragrance-free, alcohol-free product, then pat rather than rub. Give short periods without a nappy when practical, keeping the baby warm and supervised. Apply a thin protective barrier recommended for newborn skin to clean, dry skin; an emollient may help protect irritated skin. Avoid fragranced products, talc, scrubbing, and home remedies. Do not apply steroid or antifungal medicine unless a healthcare professional has advised the product and how to use it.

Contact a healthcare professional promptly if the rash is extensive, worsening, raw, blistered, bleeding, crusted, oozing, or associated with swelling. Seek urgent help if your newborn has a fever, seems unusually sleepy or difficult to wake, feeds poorly, vomits repeatedly, has fewer wet nappies, or appears seriously unwell. Ask for same-day advice if the rash is not beginning to settle with careful nappy changes and gentle cleansing, or if it keeps returning. A clinician may recommend a targeted treatment such as topical antifungal treatment when yeast is suspected.
