What should you know about extreme diaper rash?

Nappy rash commonly follows prolonged contact with urine or stool, moisture, friction, or a new product. A bright red rash involving the skin folds, small satellite spots, blisters, crusting, or open sores may suggest a different or additional cause, including yeast, bacterial infection, or contact irritation. The appearance and the child’s general condition matter more than the label alone.

Pain can make a baby unsettled during cleaning. Avoid assuming every persistent rash is simple irritation, especially when it spreads or changes despite gentle care.

Change nappies promptly, rinse with lukewarm water or use a gentle fragrance-free cleanser, and pat rather than rub. Allow short periods of air exposure when practical, and apply a protective emollient layer to clean, dry skin. Avoid fragranced wipes, talc, vigorous scrubbing, and steroid or antifungal creams unless a clinician has advised them. If yeast is suspected, topical antifungal treatment may be recommended after examination.

Contact a clinician if the rash is severe, raw, spreading, or not improving with careful nappy care. Seek urgent medical advice for fever, a baby who is unusually sleepy or difficult to settle, poor feeding, blisters, pus, rapidly spreading redness, bleeding, or signs of dehydration. A newborn or a child who seems unwell should be assessed promptly.

Ask whether the rash looks irritant, fungal, allergic, or infected; which emollient to use; how often to apply it; and what changes should prompt same-day advice.
