What should you know about extreme anal itching?

The skin can become irritated by repeated wiping, fragranced soaps, wet wipes, sweat, stool leakage or tight clothing. Eczema and contact dermatitis may cause redness, dryness, cracking or burning. Psoriasis, lichen planus and a skin yeast infection are other possibilities; scabies can cause intense itching that may also affect household contacts.

Scratching damages the skin barrier and can keep the itch–scratch cycle going. A clinician may inspect the skin and ask about bowel habits, products, nighttime symptoms, rashes elsewhere, bleeding and recent treatments.

Rinse gently with lukewarm water, pat dry and avoid scrubbing. Stop fragranced products and do not apply steroid, antifungal or numbing creams internally unless a clinician has advised it. Wear loose, breathable underwear, keep the area dry and try not to scratch; short nails can reduce skin injury.

If symptoms persist, arrange an appointment for diagnosis. The correct treatment depends on the cause, and treating presumed yeast or eczema without confirmation may delay appropriate care.

Seek prompt care for severe pain, fever, pus, spreading redness, heavy bleeding, a new lump or an open sore. Arrange a medical review for persistent itching, nighttime symptoms, weight loss, altered bowel habits or skin changes that do not settle after irritants are removed. Urgent help is appropriate for uncontrolled bleeding or rapidly worsening swelling.
