What should you know about etiology of preterm labour?

Labour before the expected time can start when the cervix shortens or opens, the uterus contracts, or the membranes rupture. Infection in the urinary or genital tract, inflammation, bleeding, and placental separation can affect these processes. Carrying more than one baby or having excess amniotic fluid stretches the uterus and may increase contraction pressure.

A previous early birth, cervical surgery, uterine differences, high blood pressure, or other pregnancy complications may contribute. In some pregnancies, clinicians cannot identify one clear trigger. Finding a cause is useful for planning care, but lack of an explanation does not make the symptoms less urgent.

Contact your maternity unit promptly for regular tightening, pelvic pressure, period-like cramps, new backache, fluid leakage, bleeding, or a noticeable change in discharge. Avoid delaying assessment to monitor symptoms alone. The team may examine the cervix, check the baby and contractions, and look for infection or membrane rupture. Depending on the situation, care may include an antenatal steroid injection or cervical cerclage planning.

Seek urgent maternity care for suspected leaking fluid, vaginal bleeding, painful regular contractions, fever, severe abdominal pain, reduced fetal movement, or sudden pressure in the pelvis. Call emergency services if there is heavy bleeding, collapse, severe breathing difficulty, or severe pain. Early assessment helps the team protect both parent and baby.

Ask what could be causing the symptoms, whether the membranes and cervix are intact, how the baby is being monitored, and which treatments could support the pregnancy if birth seems likely.
