Oestrogen receptor immunohistochemistry explained

Short answer
Oestrogen receptor immunohistochemistry is a laboratory test on tissue that shows whether cancer cells carry receptors that can respond to oestrogen.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sample
- Tissue from a biopsy or surgery
- Method
- A stain is examined by a pathologist under a microscope
- Purpose
- Helps assess whether endocrine treatment may help
What the test examines
A pathologist uses a tissue sample, usually from a biopsy or surgery, and applies a laboratory stain that attaches to oestrogen receptors. The stained sample is examined under a microscope. The report may describe the result as positive, negative or low, and may include the proportion of stained cells and staining strength. These details help the cancer team judge whether endocrine treatment could be useful.
The test does not measure the amount of oestrogen in your blood, and it does not by itself describe the whole cancer. Doctors interpret it with the tumour type, grade, progesterone-receptor result, HER2 result, scans and your health. A result can occasionally need review if the sample is small, damaged or technically difficult to interpret.
What to do with the report
Ask for a copy of the pathology report and ask the team to explain every term in it. Confirm which tissue was tested and whether another sample is needed. Discuss how the receptor result affects surgery, chemotherapy, radiotherapy or hormone therapy. If endocrine treatment is offered, ask about its purpose, expected side effects, interactions and follow-up. Keep the report with your medical records when seeking another opinion.
When to contact your care team
The test itself does not usually cause symptoms beyond those related to collecting the tissue. Contact the biopsy service for increasing redness, swelling, warmth, drainage, bleeding or fever at the sampling site. Seek urgent help for heavy bleeding, severe breathing difficulty, fainting or rapidly worsening illness after a procedure. For questions about a delayed or unclear report, contact the clinic rather than interpreting a single result alone.
Questions for your doctor
What was the oestrogen-receptor result and how was it measured? Was the sample adequate? Do I need other receptor tests or a repeat review? How does this finding change my treatment choices? Who can explain the pathology report if I have further questions?
Questions people ask
Is immunohistochemistry a blood test?
No. It is performed on a tissue sample in a pathology laboratory.
Does a positive result give the whole diagnosis?
No. It is one part of the pathology and staging information used to plan care.
