Erythema nodosum work-up

The clinician looks at the location, colour, tenderness, depth, and evolution of the lumps. Erythema nodosum often affects the shins, but the examination also checks for other skin findings and joint symptoms. The appearance can support the diagnosis, yet it does not by itself identify why the inflammation started.

Questions may cover a recent sore throat or cough, bowel symptoms, joint pain, travel, new medicines, pregnancy, and personal health history. These details help decide whether testing for infection or another inflammatory condition is useful.

Write down when the lumps began, where they appeared, and whether they changed after an illness or new medicine. Bring a list of prescription medicines, supplements, allergies, and recent treatments. Photos can help if the colour or swelling varies before the appointment.

Depending on the findings, testing may include a throat assessment, blood work, a chest evaluation, or other targeted tests. A skin biopsy is not always needed; it may be considered when the appearance is unusual or another condition needs to be excluded. Treatment should follow the findings rather than a single test result.

Arrange prompt medical review for new painful shin lumps, especially with fever, persistent cough, diarrhoea, abdominal pain, or swollen joints. Seek urgent help for breathing difficulty, faintness, rapidly worsening illness, or severe one-sided leg swelling. Do not wait for a routine appointment when these symptoms are intense or sudden.
