Erythema nodosum produces tender, deep nodules, commonly over the shins. The inflammation is centred in the subcutaneous fat rather than only at the skin surface. A clinician considers the colour, depth, tenderness, distribution, and stage of each lump, along with fever, joint discomfort and symptoms from other body systems.

When a tissue sample is needed, pathology can help show the pattern of inflammation and exclude a different panniculitis, infection, or another diagnosis. A report is interpreted together with the clinical picture; a microscopic description by itself does not identify the cause of every case.

What happens after assessment

Bring a list of recent illnesses, medicines, supplements, travel exposures, and any bowel, respiratory, or joint symptoms. Tell the clinician when the nodules appeared and whether new ones continue to form. These details help decide whether examination alone is reasonable or whether targeted blood tests, imaging, or a biopsy would add useful information.

For comfort, rest the legs, elevate them, and use cool compresses. Avoid picking, squeezing, or rubbing the nodules. Treatment focuses on comfort and any identified trigger. In selected situations, oral corticosteroid treatment may be discussed after the clinician has considered infection and other safety factors.

When to get help

Arrange a review if a lump is rapidly changing, unusually firm, ulcerated, repeatedly recurring, or accompanied by ongoing fever or unexplained weight change. Seek urgent care for severe illness, rapidly spreading redness, major one-sided leg swelling, chest pain, or difficulty breathing. These findings require prompt evaluation and should not wait for a routine pathology discussion.
