Erythema Nodosum Panniculitis Explained

Panniculitis is a descriptive term for inflammation of subcutaneous fat. In erythema nodosum, this inflammation produces deep, tender nodules, most often on the shins. The surface may look red or pink at first, then become bruise-like as each nodule fades. Unlike an abscess, the lump is not usually a pocket of pus that should be drained.

Erythema nodosum is a reaction pattern rather than a single cause. It can accompany an infection, inflammatory illness, medicine exposure, or sometimes no clear trigger. A clinician interprets the nodules alongside symptoms such as fever, joint aches, cough, abdominal upset, or changes in general health.

Keep the affected legs comfortable with rest, elevation, and cool compresses. Loose clothing prevents rubbing, while supportive shoes can reduce pressure during short walks. Avoid cutting into or repeatedly checking the nodules. Pain relief should be selected with a clinician or pharmacist if you have other illnesses, take regular medicines, or are pregnant.

Assessment may include a skin examination and targeted tests guided by your history. A biopsy is not needed in every case, but may be considered when the appearance is unusual or another form of panniculitis must be excluded. If symptoms are severe, a clinician may discuss oral corticosteroid treatment after considering the underlying cause and infection risk.

Seek medical advice for new deep, painful leg nodules, persistent fever, significant joint swelling, or symptoms that do not begin to settle. Get urgent care for rapidly worsening redness, severe one-sided leg swelling, a high fever with marked illness, chest pain, or breathlessness. Those features need assessment for causes beyond an uncomplicated inflammatory skin reaction.
