Erythema Nodosum and Itchy Legs: What to Know

Erythema nodosum often produces deep, painful, bruise-like lumps on the lower legs. Itch can occur, but prominent itch without tender nodules may fit a different skin problem, such as irritation, eczema, an insect reaction, or a medication-related rash. Scratching can damage the surface and make redness harder to interpret. The pattern, depth of the lumps, surface changes, and timing of symptoms help a clinician separate these possibilities.

Arrange an examination if itching is persistent, widespread, or joined by new lumps. Note soaps, detergents, fabrics, plants, medicines, and foods that changed before the rash appeared. Use a plain moisturiser, lukewarm water, and loose clothing; avoid fragranced products and vigorous scratching. Keep fingernails short and use a cool cloth for brief relief. A clinician can advise whether an anti-itch product is appropriate and whether the nodules need treatment. Do not assume that a steroid cream or oral corticosteroid treatment is suitable until the cause is clear.

Seek urgent help for swelling of the lips or tongue, wheezing, trouble breathing, blistering, skin peeling, or a rash with faintness. Arrange prompt care for fever, rapidly spreading warmth and redness, pus, severe pain, or difficulty walking. These warning signs need assessment regardless of whether erythema nodosum is suspected.
