What should you know about elek test diphtheria?

Short answer
The Elek test checks whether a diphtheria bacterium produces toxin, helping clinicians confirm the diagnosis and choose care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Test purpose
- Checks toxin production by a diphtheria isolate
- Sample
- Usually a clinical swab or laboratory isolate
What the Elek test means
A swab or other clinical sample may identify Corynebacterium diphtheriae, the bacterium associated with diphtheria. Finding the bacterium and showing that it releases toxin are related but different laboratory questions. The Elek test places the isolate in a culture medium with specific antibodies. A visible line where the substances meet indicates toxin and antibody have reacted. The result is interpreted by a specialised laboratory alongside symptoms, examination findings, vaccination history and other testing.
Diphtheria can affect the throat and may cause a sore throat, fever, swollen neck glands or a thick coating in the throat. A throat symptom does not by itself establish diphtheria.
What to do after testing
If diphtheria is suspected, contact with other people may need to be limited while assessment is underway. Do not wait for a laboratory result before following urgent clinical advice. A clinician may arrange antibiotics, assess close contacts and review immunisation needs. Take prescribed treatment exactly as directed, and tell the team about allergies, pregnancy, medicines and recent travel or exposure.
When to seek care
Seek urgent medical help for difficulty breathing or swallowing, drooling, a rapidly enlarging neck, faintness, severe weakness or a grey-white throat coating. These features can signal airway or systemic complications and need prompt assessment. Call ahead if diphtheria is possible so the service can reduce exposure to others.
Questions for your doctor
Ask which specimen was tested, whether toxin production was detected, when results are expected, and whether household or close contacts need assessment. You can also ask which symptoms should prompt immediate help and whether your vaccination record needs updating.

Questions people ask
Does a positive Elek test confirm toxin production?
A positive reaction supports toxin production, with the laboratory and clinician interpreting it in the full clinical context.
Should treatment wait for the Elek result?
No. When diphtheria is suspected, clinicians may begin urgent management while confirmatory testing is still in progress.
