Eczema on the back of the neck: what to know

The neck is exposed to collars, jewellery, hair, perspiration and products that run from the scalp. Heat and rubbing can intensify itching, while shampoo, conditioner, fragrance or hair dye may irritate sensitive skin. Eczema can look red, dry, scaly or thickened after repeated scratching. A rash that follows a product or sits beneath a contact point may also reflect contact dermatitis rather than atopic dermatitis alone.

Wash with lukewarm water and a mild fragrance-free cleanser, then pat dry. Apply a plain moisturiser while the skin is still slightly damp. Keep hair products off the neck, rinse shampoo thoroughly and choose soft, loose collars during a flare. Change out of sweaty clothing and avoid scratching; a cool compress can reduce the urge. If inflammation continues, a clinician can select treatment suitable for the thin skin of the neck and check whether a product allergy is involved.

Book a review if the rash lasts, repeatedly returns, spreads toward the face or scalp, or makes sleep difficult. Seek prompt care for pain, warmth, swelling, pus, spreading crusts or sudden worsening. Facial swelling, eye involvement or breathing difficulty needs urgent emergency help. Persistent darkening after the flare can be discussed as post-inflammatory hyperpigmentation.
