What should you know about eczema in the nappy area?

The nappy region is warm, damp and exposed to urine, stool, cleansing and friction. These factors can aggravate Atopic Dermatitis, although ordinary irritant nappy rash, fungal infection and allergic contact dermatitis can appear similar. Eczema may affect skin around the nappy edge and nearby creases, with dryness, redness, scaling or itch. Colour can look different after inflammation settles. A rash that involves the creases strongly or has small spots around its edge deserves a closer look.

Change nappies promptly, rinse gently with lukewarm water or use a fragrance-free cleanser, and pat the skin dry. Give short periods without a nappy when practical, use a generous protective barrier recommended for the child, and avoid talc, fragranced wipes and scrubbing. Use prescribed eczema treatment only as directed, especially on thin or broken skin. Wash hands before and after care.

Arrange medical advice if the rash lasts despite careful changes, spreads into creases, becomes very sore, cracks, oozes or forms crusts. Seek urgent help if the child has fever, seems unusually sleepy, is feeding poorly, has rapidly spreading redness or develops blisters. A clinician may need to distinguish eczema from infection before recommending treatment.
