What should you know about eczema in folds of skin?

Common sites include the neck, underarms, groin, between the buttocks and the creases behind knees or elbows. Fold skin rubs together and stays warm, while sweat and occlusive clothing can soften the surface. Eczema may look red, darker, lighter, dry or shiny depending on skin tone and stage. A fungal rash, irritant reaction or infection can look similar, particularly when the rash has a distinct edge, satellite spots, strong odour or persistent moisture.

Rinse gently, pat dry rather than rubbing, and choose loose breathable clothing. Change out of sweaty clothes promptly and avoid fragranced deodorants, harsh soaps and aggressive scrubbing. Apply a bland moisturiser to intact skin as directed. A clinician can select an anti-inflammatory treatment suitable for a fold, where stronger products may cause thinning if used incorrectly. Do not seal an uncertain rash under a thick occlusive layer without advice.

Seek review if the rash spreads, smells unusual, becomes painful, cracks deeply, oozes or develops crusts. Prompt care is also sensible when an itchy fold rash does not improve with gentle care, because fungal infection and eczema require different treatment. Urgent assessment is needed for rapidly spreading redness, severe pain, fever or feeling acutely unwell.
