Oral hairy leukoplakia often appears as a white, corrugated or hairy-looking area along the side of the tongue. It may occur when immune protection is weakened, allowing EBV activity in the mouth. The patch is generally not scraped away easily and may cause little discomfort. Its appearance can overlap with thrush, irritation, tobacco-related changes, or other mouth conditions, so a clinician should assess a persistent or changing patch rather than relying on appearance alone.

Oral hairy leukoplakia is not the same as every white patch in the mouth and is not automatically oral cancer. The wider health context matters, including medicines that affect immunity and symptoms elsewhere in the body.

Arrange a dental or medical examination if the patch lasts, spreads, becomes painful, bleeds, or changes colour or texture. The clinician may examine the mouth, review medicines and health conditions, and sometimes recommend a biopsy to clarify the diagnosis. Avoid scraping the area or applying unlabelled products. Reduce tobacco exposure and ask about smoking cessation support; keep regular oral hygiene gentle, and mention dentures, sharp teeth, or repeated rubbing.

Seek prompt assessment for a sore that does not heal, unexplained bleeding, a firm lump, increasing difficulty swallowing, persistent one-sided pain, numbness, or a neck swelling. Get urgent help if swelling affects breathing or swallowing. These warning signs need evaluation because several mouth conditions can look similar and symptoms should not be used to infer a cancer diagnosis.
