Early Pregnancy Spotting After Sex: What to Know

Pregnancy increases blood flow to the cervix, the lower part of the womb. Contact during sex can irritate this tissue and cause a small amount of pink, red, or brown spotting. Vaginal dryness, a minor surface tear, or an infection such as Chlamydia can also cause bleeding after sex. A cervical polyp or another change on the cervix may be involved.

Spotting after sex is not enough to identify the cause or to confirm that a pregnancy is continuing normally. Bleeding can occur with a threatened miscarriage, an ectopic pregnancy, or other pregnancy-related problems, so the timing, amount, colour, pain, and associated symptoms matter. A clinician may ask about your pregnancy dates and examine you; they may also arrange blood tests or an ultrasound.

Pause sex until the bleeding has stopped and a clinician has advised you about resuming it. Use a sanitary pad rather than a tampon so you can observe the amount. Note when the spotting started, whether it followed sex, its colour, and whether it is increasing. Avoid inserting anything into the vagina while you are bleeding unless a clinician has told you to do so.

Contact your obstetrician, midwife, early-pregnancy service, or another qualified clinician soon, even if the spotting is light and settles. Tell them how far along you are, whether the pregnancy has been confirmed in the womb, and your blood group if you know it. Do not drive yourself if you feel faint or unwell.

Seek emergency care now for heavy or rapidly increasing bleeding, severe or one-sided lower abdominal pain, shoulder-tip pain, dizziness, fainting, marked weakness, fever, or feeling seriously unwell. These symptoms can signal significant blood loss, infection, or an ectopic pregnancy, which needs urgent treatment.

Urgent assessment is also appropriate if you pass tissue, develop worsening cramps, or cannot reach your pregnancy care team. Do not wait for the bleeding to become heavy if pain or faintness is present.

Ask: Could the bleeding be coming from my cervix or vagina? Do I need an ultrasound or blood tests? Should I avoid sex or exercise for now? Do I need treatment for an infection? If I am Rh-negative, do I need any pregnancy-related care because of this bleeding?
