A pillow can press the cartilage around the ear or irritate a tender area of skin. Lying flat may also increase the feeling of pressure when a cold or sinus congestion affects the tube that connects the middle ear with the throat. Earwax buildup can block sound and create fullness, while a middle-ear infection may cause deeper aching, reduced hearing or fever. Pain that is clearly linked to touching or lying on the ear points toward the outer ear, but the pattern alone cannot identify the cause.
