Ear pain and stiff neck: what to know

A strained neck muscle, awkward sleeping position or jaw tension can cause pain near the ear and reduced neck movement. A cold, sinus inflammation or ear infection can also create pain in nearby areas and tender glands. Earwax may cause ear discomfort but does not explain every cause of neck stiffness.

Stiffness that is sudden, severe or associated with fever, headache, light sensitivity, vomiting, confusion or a rash is more concerning for an infection affecting the nervous system. Do not wait for ear symptoms to settle before seeking help in that situation.

If the neck feels mildly tight after a clear muscle strain and you feel otherwise well, keep movements gentle and use a warm pack for short periods. Avoid forceful stretching or manipulation. Take suitable pain relief only as directed on the label.

Seek emergency care now for a stiff neck with fever, severe headache, confusion, unusual sleepiness, light sensitivity, repeated vomiting, a rash that does not fade under pressure, weakness, trouble speaking or breathing. Urgent assessment is also needed for swelling behind the ear, facial weakness, severe dizziness or rapidly worsening ear pain.
