Ear ache without fever: what to know

Fever is only one possible sign of illness, so its absence does not identify the cause or rule out an ear problem. A blocked ear canal, pressure after a cold, or irritation from water or scratching may hurt without raising body temperature. Pain can also be referred from the jaw joint, teeth, or throat. Some respiratory illnesses, including a common cold or COVID-19, may begin with local discomfort and no fever. In a young child, changes in feeding, sleep, or behaviour can provide useful clues when describing the symptom.

Keep the ear dry and avoid cotton buds, hairpins, and home probing. Use a warm compress on the outside of the ear if it feels soothing. A pharmacist can help you choose a suitable pain reliever after checking allergies, pregnancy, age, and other medicines. Do not use ear drops if you may have a perforated eardrum or have ear discharge unless advised. Arrange a routine examination if the pain persists, affects sleep, or comes with reduced hearing.

Get urgent medical help for sudden hearing loss, severe dizziness, facial weakness, swelling behind the ear, severe headache, or fluid, pus, or blood from the ear. Seek prompt advice if a child seems very unwell, drinks poorly, or has persistent pain. Contact a clinician if symptoms worsen or do not settle, even if fever never appears; an examination can distinguish canal blockage from middle-ear or referred pain.
