Is spotting during pregnancy normal?

Spotting means a small amount of blood, often noticed on toilet paper or underwear. It is different from heavier bleeding that needs a pad. A clinician will consider how far along the pregnancy is, the colour and amount of blood, whether it is continuing, and whether you have cramps, pelvic pain, dizziness or fever.

Early in pregnancy, spotting may occur when the cervix is more sensitive and bleeds after sex or an examination. It can also come from an infection or another change in the cervix or uterus. A small bleed does not by itself show whether a pregnancy is continuing normally, so its cause cannot be confirmed from appearance alone.

Bleeding later in pregnancy has different possible explanations and should be assessed promptly. Conditions such as cervical polyps, fibroids or infection can cause bleeding, while some pregnancy complications also need urgent evaluation. Contact your obstetrician, midwife or maternity unit for personalised advice, even if the spotting is light and has stopped. Note when it began, how long it lasted, its colour, whether clots were present, and whether pain or other symptoms accompanied it. Use a sanitary pad to observe the amount; do not use tampons during pregnancy.

Follow the instructions you are given about examination, blood tests or ultrasound. If you know your blood group is Rh-negative, mention it when you call because your team may discuss whether you need specific care.

Seek urgent medical help if bleeding becomes heavy, you pass clots or tissue, or you develop severe or one-sided abdominal pain, shoulder-tip pain, faintness, marked dizziness, fever or feeling very unwell. These symptoms can signal a complication that needs prompt assessment.

Do not wait for a routine appointment if you are in late pregnancy and have bleeding, reduced fetal movement, contractions or fluid leakage. Call emergency services if you feel faint, have severe pain or cannot reach your maternity unit safely.
