Dry, Cracked, Itchy Skin on the Scrotum

The scrotal skin is sensitive and can react to fragranced soaps, detergents, sweat, friction, shaving products, or topical medicines. Scratching can deepen small fissures and make burning worse. Asteatotic eczema, atopic dermatitis, contact dermatitis, psoriasis, lichen planus, a skin yeast infection, or scabies can also affect genital skin. Hives tend to cause raised itchy areas rather than persistent cracks. The appearance, location, timing, and presence of rash elsewhere help separate these possibilities.

A yeast infection may involve moist, inflamed skin in nearby folds, while scabies often causes intense itching that is worse at night and may affect household contacts. These clues are not enough to diagnose the cause at home.

Wash with lukewarm water or a mild, fragrance-free cleanser, then pat dry without rubbing. Stop new fragranced products and avoid scratching, tight underwear, and prolonged dampness. Apply a bland, fragrance-free moisturiser to the external skin if it does not sting; do not put products inside the urethra. Avoid using steroid, antifungal, anaesthetic, or combination creams on genital skin unless a clinician has advised you, because the wrong product can irritate skin or hide diagnostic clues.

Arrange a medical review if the problem persists, recurs, spreads, or is accompanied by discharge, sores, or urinary symptoms. A clinician may inspect the skin and ask about exposures, sexual health, and symptoms in other body areas.

Seek prompt care for rapidly increasing swelling, severe pain, fever, pus, open ulcers, marked redness, or difficulty passing urine. Sudden severe testicular pain or a testicle that sits unusually high is an emergency and should not be treated as simple dry skin. Seek assessment for a new persistent lump, bleeding, or a change that does not settle.

Ask which causes fit the pattern, whether testing is needed, which cleanser and moisturiser are suitable, and how long to try the plan before returning. Mention every product used on the area, recent sexual contact, itching elsewhere, and whether anyone close to you is itchy.
