The lower eyelid contains skin, muscle, fat, and a supporting membrane. With time, the skin may become looser and the fat pads may become more visible, creating a bulge. Some people inherit prominent lower-eyelid fat or a naturally deep groove between the eyelid and cheek. That groove, called a tear trough, can make nearby fullness look more pronounced even when the amount of puffiness is modest.

Puffiness can also look stronger after poor sleep, crying, a salty meal, allergies, or rubbing the eyes. Bags that vary from morning to evening are more suggestive of fluid movement than a fixed change in eyelid structure. Dark circles, hollows, and loose upper-eyelid skin are related concerns but are not the same feature; a clinician can separate them during an examination.

Notice whether the appearance changes with sleep, allergies, crying, or eye rubbing, and mention any new medicines or skin products at an appointment. A cool compress, adequate sleep, and avoiding rubbing may reduce temporary puffiness. Treating an underlying allergy may help when itching, watering, or sneezing is present, but persistent fat pads or loose skin will not usually change through home care alone.

Treatment is chosen according to the dominant feature. Tear-trough filler may be considered for a hollow that casts a shadow, while eyelid surgery can address selected cases of protruding lower-lid fat or excess skin. These procedures require an assessment of eyelid position, skin quality, tear production, and the surrounding cheek.

Arrange prompt medical assessment if swelling is sudden, markedly one-sided, painful, red, hot, associated with fever, or accompanied by vision changes, severe headache, or trouble moving the eye. Seek urgent care for breathing difficulty or swelling of the lips and tongue. If both-sided bags are gradual and otherwise well, they are generally suitable for a routine dermatology or eye-area consultation, especially when the change persists or affects how you feel about your appearance.

Ask which feature is most responsible: fluid-related puffiness, a fat bulge, loose skin, or a tear-trough hollow. You can also ask whether allergies or another eye condition needs attention, which option matches your anatomy, what temporary effects to expect, and how treatment could affect eyelid closure, tear symptoms, or the contour beneath the eyes.
