Does topical corticosteroids treat asteatotic eczema?

Asteatotic eczema develops when very dry skin becomes irritated and inflamed. It often produces fine cracks and a characteristic network-like or craquelé appearance, commonly on the lower legs, although other dry areas can be affected. A topical corticosteroid reduces inflammatory activity in the skin, so redness, itch and discomfort may ease during a flare. It does not replace moisture, and stopping barrier care can allow dryness to return.

The strength and form of the medicine should match the body site and severity of the flare. Thin or sensitive areas need particular caution, while thicker skin may require a different approach. Scaling, oozing, crusting or a sharply expanding rash may point to another problem, such as infection or a different type of dermatitis, so the diagnosis should be checked when the appearance is unusual or treatment is not helping.

Use the prescribed corticosteroid on the inflamed areas exactly as directed, and apply a bland moisturiser frequently to dry skin. A moisturiser can be placed after the medicine if your clinician or product instructions recommend that order. Keep showers short and comfortably warm, choose a gentle fragrance-free cleanser, and pat rather than rub the skin dry. Avoid picking at cracks, since broken skin can become more irritated. If the flare is on the legs, mention swelling, poor circulation or a history of leg ulcers before choosing treatment.

Get medical advice promptly if the skin becomes increasingly painful, hot, swollen, wet, crusted or pus-filled, or if redness spreads beyond the dry rash. Seek urgent help for facial swelling, breathing difficulty or widespread blistering. Arrange a review when regular treatment does not settle the flare, the cracks keep reopening, or the rash repeatedly returns in the same area; another condition or a contributing factor may need attention.

Ask which steroid strength is suitable for each affected body area, how long to use it, and which moisturiser routine should continue between flares. Ask whether cracked or oozing skin needs a different treatment, and whether leg swelling, circulation problems or medicines could be worsening the dryness.
