Do topical calcineurin inhibitors treat inverse psoriasis?

Inverse psoriasis affects folds such as the groin, armpits, under-breast area or cleft between the buttocks. These plaques can look smooth and bright red rather than thick and scaly because moisture and friction change the surface. Topical calcineurin inhibitors reduce local immune signalling and do not carry the same skin-thinning concern as prolonged steroid use, which can make them useful in selected fold areas. However, fungal infection, irritation and other rashes can look similar, so diagnosis matters before treatment.

Apply only the prescribed thin layer to clean, dry skin and wash your hands afterwards. Keep folds gently dry, choose breathable clothing and avoid scrubbing or fragranced products. Do not use powders, antiseptics or another cream over the area unless your clinician approves them. Mild warmth or stinging may occur at first; persistent discomfort, moisture-related breakdown or worsening redness should be reported.

Seek prompt advice if the fold becomes painful, cracked, weeping, foul-smelling or rapidly more swollen, as infection or a different rash may need treatment. Arrange review when plaques do not settle, spread beyond the folds, or recur despite following the plan. Urgent help is needed for widespread blistering, facial swelling or breathing difficulty after application.

Ask whether the rash is inverse psoriasis or an infection, how long to use the medicine, how to manage moisture and friction, and whether other psoriasis sites need treatment. Confirm which products should be kept away from the fold.
