Does surgical sperm retrieval treat male factor infertility?

Male factor infertility may involve low sperm production, blockage of the tubes carrying sperm, or difficulty ejaculating. Surgical retrieval uses a needle or a small procedure to obtain sperm from the epididymis or testicle. The sample can then be used with techniques such as intracytoplasmic sperm injection, when appropriate.

The procedure does not correct every cause of infertility and does not ensure that retrieved sperm will fertilise an egg or lead to pregnancy. A fertility team considers semen results, hormone tests, examination findings and both partners' reproductive health.

Before treatment, ask how sperm production and blockage have been assessed, whether sperm will be frozen, and what happens if the first sample contains too few usable sperm. Tell the team about medicines, previous surgery, infections and genetic or hormonal conditions. The laboratory and fertility clinician should coordinate the retrieval with egg collection when needed.

Recovery varies with the technique. Follow instructions about wound care, support, lifting and sexual activity, and report concerns rather than changing the plan yourself.

Contact the clinic for increasing scrotal swelling, worsening pain, redness, wound discharge, fever or difficulty passing urine. Seek urgent help for severe pain, heavy bleeding, faintness or breathing difficulty. If no sperm are found, arrange a review to discuss the result and whether another retrieval method or a different fertility pathway is suitable.
