Does preterm labour cause severe abdominal pain in pregnancy?

Preterm labour can cause strong period-like cramps, repeated tightening, pelvic pressure, or pain in the lower abdomen and back. Severe abdominal pain can also arise from problems unrelated to labour, including infection, placental complications, or an abdominal condition. The location, onset, constant or intermittent pattern, bleeding, fluid leakage, fever, vomiting, and fetal movement all help clinicians decide how urgently to act. The symptom cannot be diagnosed safely from pain alone.

Stop activity and contact your maternity unit or obstetric team immediately for severe abdominal pain during pregnancy, particularly before term. Note when it began, where it is, whether it comes in waves, and any bleeding or fluid. Rest while waiting for advice, keep your phone nearby, and take only medicines approved for you. Do not delay care to see whether the pain passes.

Seek emergency help for severe or worsening pain, vaginal bleeding, fluid leakage, a hard or tender abdomen, fever, faintness, trouble breathing, or reduced fetal movement. Immediate assessment may include checking contractions, the cervix, membranes, infection, the placenta, and the baby. Call emergency services if you are too unwell to travel safely.
