Does preterm labour cause back pain in pregnancy?

Preterm labour refers to labour beginning before the pregnancy has reached term. Back discomfort can occur when the uterus tightens and the cervix begins to change. The pain may feel different from ordinary pregnancy strain: it can be persistent, rhythmic, or accompanied by period-like cramps, pelvic pressure, a change in discharge, or fluid leakage. Back pain in pregnancy also has common non-labour causes, so symptoms must be considered together and assessed according to how far along the pregnancy is.

Stop strenuous activity, rest on your side, and note when the pain begins, how long it lasts, and whether it repeats. Drink fluids if you can, unless a clinician has told you to restrict them. Contact your maternity unit or obstetric team promptly for new, recurring, or unexplained back pain before term. Do not drive yourself if the pain is severe or you feel unwell.

Seek urgent assessment for regular tightening, fluid leaking, vaginal bleeding, strong pelvic pressure, fever, feeling faint, or reduced fetal movement. Severe pain, worsening symptoms, or a strong sense that something is wrong also warrants immediate help. A maternity team may check contractions, the cervix, membranes, infection, and the baby; an examination is needed to tell these causes apart.
