Does perineal tear cause heavy bleeding after birth?

A perineal tear affects tissue between the vagina and anus. Small tears may ooze, while a deeper tear can involve muscle or nearby blood vessels and produce more bleeding. The birth team examines the area to judge the depth and may close it with stitches. A tear can therefore contribute to bleeding after birth, yet the amount of blood cannot identify the cause by itself.

The uterus normally tightens after the placenta is delivered. If it does not tighten enough, or if placental tissue remains, bleeding may come from inside the uterus rather than the perineum. The team may assess both areas at the same time.

Report bleeding to your midwife, obstetrician or maternity unit straight away, especially if it is increasing or accompanied by clots, light-headedness or unusual weakness. Note when it started and whether pain or swelling at the perineum is worsening. Do not assume a tear explains the bleeding until a clinician has examined you. After discharge, follow your maternity service's urgent-care instructions.

Call emergency services for heavy or fast-flowing bleeding, fainting, severe dizziness, breathlessness, chest discomfort, confusion, a pounding heartbeat, or cold sweaty skin. Get urgent review for severe perineal pain, a rapidly enlarging swelling, wound separation, fever, or foul-smelling discharge. These symptoms may need immediate examination and treatment.

Ask how deep the tear is and whether it has been fully repaired. Ask whether the uterus is contracting normally, whether the placenta is complete, and what amount of bleeding should prompt help. You can also ask how to clean the area, recognise infection, and arrange a wound check.
