Perimenopause is the transition before menopause, and its pattern can vary from one cycle to the next.

What a missed period can mean during perimenopause

During perimenopause, the ovaries may not release an egg in every cycle. As hormone levels change, the uterine lining may build and shed on a different schedule, which can lead to a late or missed period. Other changes may occur alongside this pattern, such as hot flushes, night sweats, sleep difficulty, vaginal dryness, or mood changes. These symptoms are not required for perimenopause, however, and a missed period has other possible explanations.

Pregnancy can still occur while periods are changing, because ovulation may continue unpredictably. Stress, major changes in exercise or weight, thyroid conditions, some medicines, and other reproductive health conditions can also affect bleeding. A missed period by itself cannot identify the cause.

What to do after a missed period

If pregnancy is possible, take a home pregnancy test according to its instructions and contact a healthcare professional if the result is positive or unclear. Track the first day, length, flow, and symptoms of each period in a calendar or app. This record can show whether cycles are becoming less predictable and gives your clinician useful context.

Arrange a routine appointment if missed periods continue, bleeding becomes difficult to manage, or symptoms interfere with sleep, work, sex, or daily activities. A clinician may review your cycle history, medicines, pregnancy possibility, and other symptoms before deciding whether an examination or tests are appropriate.

When to seek care promptly

Seek urgent medical care for severe or one-sided lower-abdominal pain, fainting, marked dizziness, shoulder-tip pain, or heavy bleeding, especially if pregnancy is possible. These symptoms can need prompt assessment. Contact a clinician soon for bleeding after sex, bleeding between periods, new pelvic pain, fever, or a bleeding pattern that is markedly different for you.

Do not assume that every missed period is perimenopause. If periods stop before the expected menopausal transition, or if you have health conditions or take medicines that affect cycles, ask for an individual assessment.
