Does pelvic inflammatory disease cause pelvic pain?

Pelvic inflammatory disease is an infection affecting the upper reproductive organs. It may cause pain low in the abdomen or pelvis, sometimes on both sides. Pain can be persistent, cramp-like, or more noticeable during sex. Other features may include unusual vaginal discharge, bleeding between periods or after sex, fever, or discomfort when urinating. Some people have mild symptoms, so the absence of severe pain does not rule it out. Pelvic pain by itself does not establish that pelvic inflammatory disease is present; infections of the urinary or reproductive tract and other pelvic conditions can feel similar.

Arrange a clinical assessment if pelvic pain is new, ongoing, or occurs with discharge, fever, bleeding, or pain during sex. A clinician may ask about symptoms and sexual health, examine the abdomen and pelvis, and offer tests for infections or pregnancy when relevant. If pelvic inflammatory disease is suspected, treatment is usually started promptly because untreated infection can spread and may affect future reproductive health. Take prescribed medicine exactly as directed and avoid sex until a clinician says it is safe; partners may also need assessment.

Seek urgent medical help for severe or rapidly worsening pelvic pain, fainting, repeated vomiting, a high fever, heavy vaginal bleeding, or feeling very unwell. Get prompt advice as well if you are pregnant or could be pregnant and have pelvic pain, because pregnancy-related problems need separate assessment.
