Does ovarian torsion cause pelvic pain?

Ovarian torsion happens when an ovary twists around the tissues supporting it. The twist can affect blood flow and trigger marked pelvic or lower abdominal pain. Pain may come in waves or remain steady, and nausea or vomiting can occur. A person may also feel tenderness, but symptoms vary and pelvic pain can have many other causes.

Because the ovary may be under strain while twisted, the condition is treated as urgent. A clinician may use an examination, ultrasound, and other tests to assess the ovary and rule out similar problems. Tests cannot always settle the diagnosis without specialist assessment.

Sudden or severe pelvic pain should not be managed at home while waiting for it to pass. Contact urgent medical services or attend an emergency department, particularly if the pain is one-sided or accompanied by vomiting, faintness, fever, or increasing tenderness. Tell the clinician when the pain started and whether you know of an ovarian cyst or recent fertility treatment.

Do not drive yourself if you feel faint or severely unwell. Bring your medicines and mention possible pregnancy, previous ovarian surgery, or known cysts, as these details can change the assessment.

Seek immediate care for sudden intense pelvic pain, pain with repeated vomiting, collapse or faintness, fever, or a rigid or very tender abdomen. Prompt assessment is needed because untreated torsion can threaten the ovary and because other urgent abdominal conditions can look similar.
