Does Osteoporosis Cause Persistent Bone Pain?

Osteoporosis reduces bone strength and may cause no symptoms until a fracture occurs. A spinal compression fracture can produce ongoing back pain, height loss or a change in posture; fractures elsewhere may cause pain, tenderness and difficulty using the affected area. Persistent pain still needs an assessment because osteoporosis is not the only possible explanation.

Weakened vertebrae can collapse gradually or after a minor movement, causing pain that may worsen with standing, walking, bending or lifting. A fracture in the hip, wrist or another bone usually causes more sudden pain and may limit movement. Muscle strain, arthritis, infection and other conditions can cause similar symptoms, so the pain pattern does not establish the diagnosis.

A clinician may review fracture history, medicines, falls, nutrition and other health conditions. Examination and imaging can look for a fracture, while a bone-density test may help estimate bone strength and guide prevention planning.

Book a medical appointment for persistent bone or back pain, a new stooped posture, or pain after a minor fall or lifting movement. Until assessed, avoid activities that sharply increase pain, use safe movement, and ask about suitable pain relief rather than relying on prolonged self-treatment. Tell the clinician about previous fractures, long-term steroid use, smoking, falls and medicines that may affect bone strength.

Seek urgent care after a fall or sudden movement if pain is severe, the limb looks deformed, or you cannot stand, walk or use it normally. Emergency assessment is also needed for back pain with new leg weakness, numbness around the groin, or loss of bladder or bowel control. These symptoms can signal a serious fracture or pressure on nerves.

Ask: Could this pain be from a fracture? Do I need imaging or a bone-density test? How can I move safely while painful? Which pain medicines fit my health history? Should I be assessed for fall risks or causes of weakened bone? What treatment and follow-up could lower the chance of another fracture?
