Does oral corticosteroid treatment treat pyoderma gangrenosum?

Pyoderma gangrenosum is an inflammatory condition that can cause a painful ulcer with a ragged or undermined edge. The inflammation may worsen after injury or an unsuitable procedure, so diagnosis and treatment should be guided by an experienced clinician. Oral corticosteroids can suppress the immune inflammation and may be used when disease is active or progressing.

The medicine does not replace wound care. Dressings, pain support, infection assessment, and treatment of an associated condition may all be part of the plan. A dermatologist may consider another immune-directed treatment when corticosteroids are unsuitable, insufficient, or need to be reduced.

Seek dermatology review for a painful ulcer that enlarges, develops a dusky edge, or fails to heal. Do not self-treat with leftover steroids, and do not arrange a biopsy or procedure without telling the treating team that pyoderma gangrenosum is suspected. Keep the dressing plan gentle and bring information about bowel, joint, blood, or inflammatory symptoms that may point to an associated illness.

Obtain urgent help for rapidly enlarging ulceration, severe pain, fever, faintness, confusion, heavy bleeding, or inability to keep fluids down. Call promptly if the wound develops spreading redness, warmth, foul drainage, or a sudden change in appearance. These signs can reflect infection or rapid inflammatory activity and need examination.

Ask how the diagnosis was established, how the ulcer will be dressed, what improvement should look like, and whether another immune treatment is needed. Ask which procedures to avoid during active disease and how the steroid will be adjusted safely.
