Does antihistamine treatment treat heat hives?

Heat hives, also called cholinergic urticaria or exercise hives, can appear as small itchy wheals when body temperature rises. Exercise, hot showers, emotional stress, fever, or a warm room may trigger them. An antihistamine blocks histamine activity and may make the reaction milder, but it may not prevent every flare. The pattern can resemble other forms of urticaria, so a diagnosis depends on the timing and appearance of the rash.

Heat hives usually settle as the body cools, but repeated episodes can interfere with exercise, work, sleep, or confidence. Keeping a simple trigger diary can reveal patterns that are easy to miss.

Take an oral antihistamine only as directed by a clinician or pharmacist, especially if you take other medicines or need to stay alert. Cool the skin gradually, wear breathable layers, and avoid sudden overheating while you learn your triggers. Do not test a suspected trigger alone if previous episodes included breathing symptoms or faintness. Record the activity, temperature, onset, duration, and any symptoms beyond the skin.

Stop the triggering activity and seek emergency help for wheezing, throat tightness, trouble breathing or swallowing, dizziness, faintness, or swelling of the tongue or lips. These symptoms can be more serious than a skin-only flare. Arrange a medical review when episodes are new, severe, frequent, occur without warmth or exertion, or leave bruising or lasting marks.
