Does oral antifungal treatment help with thickened nails?

Fungal nails can become thicker, brittle, crumbly, or difficult to trim. Oral treatment reaches the nail as it grows and may be considered when infection affects the nail substantially, keeps returning, or has not responded to suitable local care. Thickening itself is not proof of fungus: tight footwear, repeated injury, psoriasis, and other nail disorders can produce a similar appearance.

A clinician may inspect the nail and take a clipping or scraping before choosing treatment. The existing thick section does not disappear instantly because nail tissue grows slowly. Trimming or professional thinning may make the nail more comfortable while the underlying cause is addressed.

Have persistent thickening assessed, especially if several nails are changing or walking and footwear are uncomfortable. Keep feet dry, wear roomy shoes, and avoid sharing nail tools. Tell the clinician about liver disease, regular medicines, pregnancy or plans for pregnancy, and any earlier antifungal reaction. Do not borrow tablets or stop a prescribed course without guidance; ask whether follow-up or testing is needed.

Arrange prompt medical review for severe pain, redness spreading around the nail, warmth, swelling, pus, or a nail that suddenly lifts. A new dark streak or pigment extending onto nearby skin also needs assessment. Seek emergency help for breathing trouble or facial swelling after taking medicine.

Ask whether the thickening looks fungal, whether a sample should be tested, what improvement will look like, and whether trimming or topical care can help comfort. Discuss medicine interactions, possible side effects, and the point at which another diagnosis should be considered.
