Does hypothyroidism cause muscle cramps?

Short answer
Muscle Cramps is a common presentation of Hypothyroidism, although cramps can also have other causes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible thyroid link
- Low thyroid hormone can contribute to muscle stiffness and cramping.
- Useful record
- Note the muscle affected, timing, triggers, and associated symptoms.
What the connection can mean
When thyroid hormone levels are low, muscles may become more prone to stiffness, aching, fatigue, and involuntary tightening. Cramps may occur in the legs or feet, during activity, or at rest. Low fluid intake, strenuous exercise, mineral imbalance, medicines, nerve problems, and circulation issues can cause similar symptoms, so the symptom alone cannot identify the cause.
A clinician may ask when the cramps began, which muscles are affected, whether weakness or swelling is present, and whether you have other features of an underactive thyroid such as feeling unusually cold, dry skin, constipation, or low energy.
What you can do now
Keep a note of the timing, location, triggers, and duration of each cramp. Gentle stretching and movement may relax a tightened muscle; regular fluids and gradual warm-up can also help when exercise is a trigger. Do not start thyroid medicine or mineral supplements without medical advice.
Arrange a primary-care assessment if cramps keep returning, disturb sleep, or occur with tiredness, cold sensitivity, constipation, or changes in weight. Thyroid blood tests and an examination can help separate hypothyroidism from other explanations.
When to seek care
Seek urgent help for a cramp with sudden severe weakness, inability to walk, a new one-sided change, chest pain, trouble breathing, confusion, or fainting. Prompt assessment is also sensible for a painful swollen limb, dark urine after severe muscle symptoms, or cramps that follow a new medicine.
Questions for your doctor
Could my symptoms fit hypothyroidism or another muscle problem? Which blood tests are appropriate? Could any medicine, activity, fluid change, or mineral issue be contributing? What should I do if the cramps worsen?

Questions people ask
Can muscle cramps be the only symptom of hypothyroidism?
They can occur alone, but cramps are more informative when considered with symptoms, examination findings, and thyroid blood tests.
Should I take magnesium for thyroid-related cramps?
Ask a clinician first, because supplements may be unsuitable and cramps may have a different cause.
