Does hypoglycaemia cause irritability?

How low blood sugar can affect mood

When glucose drops, the body releases stress hormones to restore energy. This can cause irritability, anxiety, restlessness, sweating, trembling and a racing heartbeat. The person may seem short-tempered, confused or unable to focus, and may not recognise the change themselves.

Irritability has many possible causes, such as poor sleep, pain, stress, illness or another medicine effect. A mood change is therefore a sign to assess alongside food intake, diabetes treatment and other symptoms, not a diagnosis on its own.

What you can do

If the person is awake and can swallow, check glucose when possible and follow their written low-glucose plan. Give the recommended quick-acting glucose, then reassess as instructed. A follow-up meal or snack may be needed to prevent symptoms returning.

Speak calmly and reduce stimulation. Do not argue about the behaviour or let the person drive. After recovery, write down the episode, including meals, activity, alcohol, medication timing and glucose readings, so a clinician can look for a pattern.

When urgent help is needed

Call emergency services if irritability progresses to severe confusion, aggression that cannot be managed safely, seizure, collapse, inability to swallow or unconsciousness. Never put food or drink in the mouth of someone who is not fully alert.

Arrange a medical review for repeated mood changes, episodes without diabetes treatment, or symptoms that continue after glucose is corrected. Prompt review is especially useful after a new prescription or a change in dose.
