Does heart failure cause swollen ankles?

When the heart cannot pump blood as effectively, pressure can build in the veins. The body may also retain salt and water, allowing fluid to move into tissues. This often causes puffiness around both ankles and feet, sometimes extending up the legs. Swelling may be more noticeable after standing and may improve after resting with the legs raised. Heart failure can also cause breathlessness, unusual tiredness, a cough, or rapid weight change from fluid, but symptoms vary.

Ankle swelling is not specific to heart failure. Long periods of sitting or standing, vein problems, some medicines, kidney or liver conditions, and a local injury can also contribute. A clinician considers whether the swelling affects one or both sides, when it began, and what other symptoms are present.

Arrange a medical assessment if new swelling persists, returns, or occurs with breathlessness. Until you are assessed, avoid prolonged standing, support your legs when sitting, and keep a note of changes in swelling, breathing, and weight. Do not start water tablets or sharply restrict fluids without medical advice, because the right plan depends on your heart and kidney health. Follow any existing heart-failure plan and take prescribed medicines as directed.

Seek urgent help for severe or rapidly worsening breathlessness, chest pressure, fainting, confusion, blue or grey lips, or coughing up pink frothy sputum. Sudden swelling in one leg with pain, warmth, or breathlessness also needs prompt assessment. These signs can indicate a serious heart or circulation problem and should not be managed at home.

Ask what may be causing the swelling, whether you need heart or kidney checks, how to monitor fluid changes, and which symptoms should trigger urgent help. Bring a list of medicines, including non-prescription painkillers, because some can worsen fluid retention.
