Does surgical excision treat xanthelasma?

Xanthelasma appears as yellowish cholesterol-containing deposits, usually on or around the eyelids. Excision physically removes a selected deposit and may be considered when it is raised, bothersome or cosmetically concerning. The eyelids have delicate skin and important structures, so the amount removed must be planned carefully. Other options may be discussed depending on the deposit’s depth and position. Even after successful removal, similar deposits can return or appear elsewhere.

Have the area examined before choosing treatment, particularly if the appearance is new, one-sided, inflamed or unusual. Ask whether a blood test for cholesterol or other health factors is appropriate. Share any history of dry eyes, eyelid surgery, vision problems or medicines that affect bleeding, and follow the specific instructions for protecting the wound.

Arrange review if a deposit changes rapidly, becomes painful, ulcerates or affects eyelid movement. After treatment near the eye, seek urgent advice for vision changes, severe swelling, heavy bleeding, increasing redness, discharge or pain that is getting worse.

Ask whether excision suits the deposit’s location, what alternatives are available, how eyelid movement and closure will be protected, where scarring may occur, and how recurrence or new deposits will be monitored.
