Why heels crack

Heel fissures develop when dry, thickened skin loses flexibility and splits under pressure as the heel bears weight. Open footwear, prolonged standing, dry air, hot water, harsh cleansers, and some skin conditions can contribute. An emollient reduces water loss and makes the outer layer more supple, helping the edges of a shallow crack move less with walking. It works gradually with repeated use rather than sealing every fissure immediately.

Cracking can also occur with eczema, psoriasis, fungal infection, or other medical problems. Very deep fissures may bleed and can allow bacteria to enter, so the appearance and symptoms matter.

A heel-care routine

After a short lukewarm wash, pat the heels dry and apply a thick, fragrance-free ointment or cream while the skin is still slightly damp. Cover with clean cotton socks overnight. Wear supportive, closed-back footwear that limits heel movement and avoid walking barefoot on hard surfaces while cracks are open. Do not cut thick skin with a blade. If the skin is intact around a shallow fissure, gentle filing after bathing may reduce scale; stop if the area becomes tender or bleeds. Keep shared files and towels clean.

When to seek care

Arrange prompt care for increasing redness, warmth, swelling, pus, fever, spreading pain, or a crack that is deep, bleeding, or preventing normal walking. Seek advice before treating at home if you have diabetes, reduced sensation, poor circulation, or a weakened immune system. Persistent heel scaling or itch may need assessment for a fungal or inflammatory condition rather than moisturiser alone.
