Does custom orthotics treat hammer toe?

Short answer
Custom orthotics may reduce pressure and improve comfort with hammer toe, but they do not reliably straighten a toe that has become fixed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible benefit
- Less forefoot and shoe pressure
- Fixed toe
- Usually needs more than an insert
- Monitor
- Redness, corns or skin breakdown
What the orthotic can change
A hammer toe bends at one or more toe joints, which can make the top of the toe rub against a shoe or create a painful area beneath the forefoot. An orthotic can cushion and redistribute load so the toes are not asked to grip as hard during walking. This may help when pressure under the ball of the foot contributes to symptoms. It does not remove a contracture, correct a rigid joint position or make a prominent area disappear.
Flexible and fixed hammer toes behave differently. A clinician can check whether the toe can be gently straightened, inspect the skin and assess the way your foot loads. The device must fit with your shoes; a bulky insert or narrow toe box can increase crowding rather than relieve it.
Ways to reduce irritation
Choose shoes with a high, wide toe box and avoid footwear that presses down on the bent toe. A clinician may suggest a toe pad, stretching or strengthening when the toe remains flexible, alongside an orthotic if the foot mechanics support it. Build up wear of a new device gradually and inspect for redness that does not settle. Persistent pain despite footwear and support deserves reassessment, because the source may be a joint, tendon or forefoot problem requiring a different plan.
When to seek care
Seek prompt advice for an open sore, spreading redness, sudden swelling, severe pain, loss of sensation or inability to walk. Arrange a foot assessment if the toe is becoming less flexible, repeatedly develops corns, or interferes with ordinary shoes. People with diabetes or reduced circulation should have new pressure spots checked early.
Questions for your doctor
Is my hammer toe flexible or fixed? Where is the pressure coming from when I walk? Would an orthotic, toe pad or exercise be most useful? How can I tell whether a shoe is safe for the toe?
