Does chemotherapy treat HER2-positive breast cancer?

Chemotherapy acts on rapidly dividing cells, while HER2-targeted medicines act on signals linked to the HER2 protein. Using these approaches together can attack the cancer in different ways. The recommended combination depends on the cancer's extent, tumour features, previous care and your health.

Chemotherapy and HER2-targeted therapy may be offered before breast surgery to shrink the tumour and provide information about its response. They may instead follow surgery to address cancer cells that could remain. When cancer has spread beyond the breast and nearby lymph nodes, drug treatment can be used to control it and reduce symptoms.

Your pathology results establish HER2 status and help shape the drug plan. Heart function may be checked because some HER2-targeted medicines and certain chemotherapy drugs can affect the heart. Monitoring allows the team to adjust timing or treatment if concerns arise.

Share your full medication and supplement list, pregnancy plans, heart history and any previous reactions to medicines. Ask whether you need a heart scan, blood tests or a device for intravenous treatment. Discuss fertility preservation before chemotherapy if future pregnancy matters to you.

During treatment, note fever, diarrhoea, vomiting, breathlessness, swelling, numbness and changes around the infusion site. Take supportive medicines exactly as directed and use the oncology team's contact route if symptoms are not controlled.

Contact the cancer unit urgently for fever, chills, persistent vomiting or diarrhoea, inability to keep fluids down, unusual bleeding, or redness and pain around an intravenous line. Infection can become serious when white blood cells are reduced.

Get emergency help for severe breathing difficulty, chest pressure, fainting, sudden facial or throat swelling, or signs of a severe infusion reaction. New breathlessness, ankle swelling or a racing heartbeat also deserve prompt contact with the treating team because they can signal heart strain.

Ask which chemotherapy and HER2-targeted medicines are planned, whether treatment comes before or after surgery, and what findings could change the schedule. Confirm how heart health will be monitored, which effects need same-day advice and what support is available for fertility or menopausal symptoms.
